اثربخشی طرحواره درمانی بافتاری بر نشانگان اضطراب و اجتناب تجربی بیماران مبتلا به اختلال اضطراب تعمیم‌یافته

چکیده

زمینه: هدف پژوهش حاضر تعیین اثربخشی طرحواره درمانی بافتاری بر نشانگان اضطراب و اجتناب تجربی بیماران مبتلا به اختلال اضطراب تعمیم ­یافته بود.
روش: در این مطالعه نیمه ­آزمایشی از طرح پیش ­آزمون-پس­ آزمون تک ­گروهی با دوره پیگیری دو­ماهه استفاده شد. جامعه آماری شامل تمام افراد مبتلا به اختلال اضطراب فراگیر بود که از 1401 تا 1403 به مراکز مشاوره دانشگاه بیرجند و کلینیک همراه همیشگی در شهر بیرجند مراجعه کرده بودند. نمونه مورد نظر نیز شامل 23 نفر از این بیماران بودند که به روش نمو­نه­ گیری هدفمند انتخاب شدند. شرکت­ کنندگان به مدت 16 جلسه (60 دقیقه ­ای) طرحواره درمانی بافتاری فردی را دریافت کردند. برای جمع­ آوری داد­ه ­ها از مصاحبه بالینی ساختاریافته براساس DSM-5 (SCID)، مقیاس کوتاه اختلال اضطراب فراگیر (GAD-7)، پرسشنامه­ های اضطراب بک (BAI) و پذیرش و عمل (AAQ-II) استفاده شد. داده­ها با آزمون تحلیل واریانس مکرر و شاخص تغییر پایا تحلیل شد.
یافته ­ها: نتایج این مطالعه نشان داد که طرحواره درمانی بافتاری به طور معناداری نشانگان اضطراب و اجتناب تجربی را در بیماران مبتلا به اختلال اضطراب تعمیم ­یافته کاهش می­دهد (0/05 >p). شاخص تغییر پایا نیز برای نشانگان اضطراب و اجتناب تجربی بالاتر 1/96 بود. نهایتاً، بهبود نشانه ­ها در دوره پیگیری نیز معنادار بود
(0/05>p).
نتیجه ­گیری: طرحواره درمانی بافتاری در کاهش نشانگان اضطراب و اجتناب تجربی مؤثر است و می­توان از آن به عنوان یک رویکرد درمانی در بهبود بیماران مبتلا به اختلال اضطراب تعمیم­یافته استفاده کرد.

Background: The aim of the present study was to determine the effectiveness of contextual schema therapy on anxiety symptoms and experiential avoidance in patients with generalized anxiety disorder.
Method: This quasi-experimental research employed a single-group pretest-posttest design with a two-month follow-up. The statistical population included all individuals with generalized anxiety disorder who had been referred to the counseling centers of the University of Birjand and Hamrh-E Hamishegi Clinic in Birjand from 2022 to 2024. The sample included 23 patients selected through purposive sampling. Participants received individual contextual schema therapy for 16 sessions (60 minutes). To collect data, The Structured Clinical Interview for DSM-5 (SCID), the Generalized Anxiety Disorder Assessment-7 (GAD-7), the Beck Anxiety Inventory (BAI), and the Acceptance and Action Questionnaire–II (AAQ-II) were used. The data were analyzed using repeated-measures analysis of variance and the Reliable Change Index (RCI).
Results: The results showed that contextual schema therapy reduces anxiety and experiential avoidance symptoms in patients with generalized anxiety disorder significantly (p< 0.05). The Reliable Change Index on anxiety and experiential avoidance symptoms was also significant. Finally, the improvement of symptoms during the follow-up was also significant (p< 0.05).
Conclusion: Contextual schema therapy is effective in reducing anxiety and experiential avoidance symptoms and can be used as a therapeutic approach in improving patients with generalized anxiety disorder.
